Appointment Confirmation and Next Steps

Thank you for scheduling your appointment with us! Your appointment has been successfully confirmed, and we look forward to seeing you soon. Step 1:

Appointment Confirmation

After you schedule your appointment, you will receive a confirmation message to ensure your appointment has been successfully booked. This confirmation will include the date, time, location, and provider you are scheduled to see. If you do not receive this confirmation within 24 hours, please contact our office to verify your appointment. This step helps us ensure that your time is reserved and that we’re fully prepared for your visit.

Step 2:

New Patient Paperwork and Consents

Once your appointment is confirmed, you will receive an email with your new patient paperwork and consent forms. Completing these forms in advance helps streamline your check-in process and ensures we have all the necessary information to provide you with the best care. These forms typically include personal health history, current medications, and our clinic policies. We ask that you complete and submit these forms at least 24 hours before your scheduled appointment to avoid delays and maximize your appointment time.

Step 3:

Specialty Appointment Forms

If you scheduled a specialty appointment, you will also receive a separate text message and email with condition-specific questionnaires or forms tailored to your needs. These forms help us gather detailed information about your symptoms, medical history, and treatment goals, allowing us to provide a more personalized and efficient consultation. Please complete these specialty forms prior to your appointment.

Step 4: 

Special Note regarding Insurance

Please note that we are an out-of-network provider for all insurance plans and have opted out of Medicare. Payment is due at the time of service, but we can provide an itemized receipt upon request if you wish to seek out-of-network reimbursement from your insurance provider (excluding Medicare). Many of our patients also use HSA or FSA funds for services, though we recommend confirming eligibility directly with your insurance carrier. We look forward to seeing you and providing the highest level of individualized care.

Step 5: 

Day-of Appointment Instructions

When you arrive, parking is conveniently available in front of the building. The clinic is located on the second floor in Suite 204 and is accessible via stairs only. When you reach the suite, please feel free to come right in—there’s no need to knock. We look forward to welcoming you and providing the highest level of individualized care.
